1. I am required to provide specific information as part of this service and am aware that my failure to give complete and accurate responses could lead to injury.
2. I am aware that I must inform the massage therapist immediately of any adverse changes occurring during the massage such as dizziness, discomfort, pain etc. I am aware that failure to so inform the massage therapist could lead to possible injury or illness.
3. I am physically capable of, and there is no medical and/or any other reason to prevent me from taking part in this service.
4. I am aware that I must allow the massage therapist to adjust any massage chair or equipment prior to my use.
5. My signature on the booking sheet is my acknowledgement that I understand and accept the above terms.
Privacy
Health at Work may also compile statistical data reports on industry or health trends at its discretion. Health at Work's privacy policy is in compliance with Australian Government privacy legislation.
